During the processing of your Medicare enrollment application, many questions may arise where the analyst needs to contact a knowledgeable individual who can assist in getting the answer or documentation needed to complete processing.

The Contact Person Information section of the CMS-855S should be completed with the name, phone number and email address of an individual who is knowledgeable of the business and available to the analyst to answer any questions. Often this section is left blank and the analyst only has the name of the authorized official who is not always available. Not being able to contact someone for the required information can result in extended processing times or rejection of the application.

Also, please be aware this individual can only be contacted during the processing of the application. Once processing is complete only individuals listed on the supplier enrollment may receive or be contacted for additional information.
